It was fine for the the last two times that I was on it, just slow. If it was stuck in one gear for the full route, that suggests to me that the driver had used gear selector buttons to hold it in one gear. There's R N D 2 1 - reverse, neutral, drive -1 holds whatever gear you're in, and 2 lets it change up one gear only, for example a change from 1 to 2 and then no more if I remember correctly. 
Wouldn't that overtime damage the engine thomas?
Over time it most likely would yes, however I've seen it happen before!  They're supposed to be used in situations such as hill climbing apparently, however some drivers use them all the time. I suppose you have to on things such as Arrows, which tend to be not the fastest of all buses haha! :P More modern buses though such as B7RLEs and B9TLs for example don't have the 1 2 buttons though, they only have R N D from what I've seen!
