There will always be failures.
I was working at Manchester Piccadilly yesterday and noted that the 1240 MIA-LIV was cancelled but 319367 was on the 1540 and 1840 legs of the diagram.
319362 was working the MAN-MIA shuttle.

1240 Man Air-Lime St started at Man Ox Rd due to no driver as they were caught up in signalling problems at Preston

Yes I may have put a curse on it as 319368 was at the rear of platform 1 with not in service on the front and lights off at about 8:00pm last night.

It wasn't a failure it was heading to the depot

There was also an incident in the Earlestown area last night involving an incoming service from Manchester Airport.
